Why Imaging Doesn’t Show Pain

Your MRI can be completely normal. Your pain can still be completely real.

That sounds like a contradiction.

It isn’t.

A scan can show a disc bulge.

A CT can show arthritis.

An X-ray can show degeneration.

But none of them can show pain.

And that distinction matters.

Because one of the most frustrating experiences for someone living with chronic pain is hearing:

“Your scan is normal.”

When what they hear is:

“There’s nothing wrong with you.”

Those are not the same thing.

An MRI Shows Structure. It Doesn’t Show Pain.

Radiological imaging is extraordinarily good at showing anatomy.

MRI can show discs, nerves, joints and soft tissues.

CT can show bone and structural changes in remarkable detail.

X-rays can show alignment, fractures and changes in joint spaces.

But pain isn’t simply a picture of damaged tissue.

Pain is an experience produced by the nervous system.

Sometimes that response is pain.

Sometimes it isn’t.

This is why two people can have almost identical MRI findings and completely different experiences.

The image hasn’t failed.

The image is simply answering a different question.

It is showing structure, not suffering.

A standard MRI wasn’t designed to show these processes.

You can have severe chronic pain while your structural imaging looks reassuring.

That isn’t evidence that the pain isn’t real.

It is evidence that pain is more complicated than anatomy.

This matters across many chronic pain conditions.

  • Persistent low back pain.
  • Fibromyalgia.
  • Chronic pelvic pain.
  • Endometriosis associated pain.
  • Chronic headache.
  • Neuropathic pain.
  • Complex regional pain syndrome.
  • Persistent post surgical pain.

In each of these conditions, there may be a disconnect between what the tissues look like and what the nervous system is doing.

The original injury may have healed.

The surgery may have been technically successful.

The structural abnormality may be relatively minor.

And yet the person still hurts.

Sometimes the next step isn’t another scan.

It is understanding the nervous system.

So What Does a “Normal” Scan Mean?

It means something important.

But perhaps not what you think.

A normal scan may rule out certain structural causes of pain.

That is valuable.

It may make dangerous pathology less likely.

That is reassuring.

But it does not mean:

“You have no pain.”

It does not mean:

“It’s all in your head.”

And it does not mean:

“Nothing can be done.”

It means we may need to ask a different question.

If the structure doesn’t adequately explain the symptoms, what else is happening?

Pain is the nervous system’s protective response to perceived threat.

Tissue damage can be part of that threat.

But it isn’t the only input.

This explains why pain can exist without obvious structural damage.

And why structural damage can exist without pain.

It explains why someone can have severe symptoms with a “normal” MRI.

And why someone else can have a frightening-looking MRI and feel completely fine.

The scan is not lying.

The patient is not imagining it.

They are simply describing two different parts of the same system.

Imaging Is a Tool. Not a Verdict.

The goal is to understand why this person hurts.

Sometimes the answer is structural.

Sometimes it is neurological.

Often, it is a combination.

And sometimes the most important finding on an MRI is the one that doesn’t explain the pain.

Because when the scan doesn’t tell the whole story, that’s not the end of the investigation.

It’s the beginning of a better question.
